Central register of precautionary measures Entry

Description

A health care proxy and/or care directive can only be effective if it is found in an emergency.

In the event of an emergency, the registered details can be retrieved by guardianship courts around the clock via secure online access. If the emergency occurs very suddenly and you are no longer able to express yourself, the guardianship court can thus clarify whether there is a health care proxy or a care directive and - if you have provided information on this - where the document is kept. This can prevent legal guardianship from being ordered.

You can register the following data electronically in the Central Register of Provisions of the Federal Chamber of Notaries:

Data of the grantor of power of attorney or of the disposing person
Data of the authorized representative or proposed guardian
Date on which the document was drawn up
Place where the document is kept
Information on the purpose for which the power of attorney was granted or the care directive was drawn up
Details of special instructions and wishes, including whether a living will has also been drawn up.

Please note that the Central Precautionary Register does not keep the document (including the copy) in which the health care proxy or care directive is contained.

When you make a new registration, you will receive a personal ZVR card in addition to the confirmation of registration. On this card, you can write down the name of the person granting the power of attorney as well as the names and telephone numbers of up to two trusted persons.

You can change, add to or delete entries in the Central Pension Register at any time.

For institutional users of the register (for example, notaries, attorneys, guardianship associations or guardianship authorities registered with the Central Register of Advance Care), special conditions apply for registrations with the Central Register of Advance Care. If, for example, you have had your health care proxy and/or care directive notarized by a notary, he or she can also register it. The notary does not receive any fees of his own for the registration and only charges you the lower registration fee in addition to the notarization fee.

EUR 8.50 - 16.00 for the registration of the first authorized representative
EUR 2.50 - 3.00 for each additional authorized representative

Process flow

You can apply for registration in the Central Precautionary Register yourself online or in writing.

If you want to apply for registration online:

Go to the website of the Central Precautionary Registry and follow the instructions.
After you apply for registration online, the Central Precautionary Registry will send you a data control sheet and an invoice. From the data control sheet, you can see the data to be entered and make any necessary corrections.
Once you have paid the applicable fee, your health care proxy or care directive is registered in the Central Care Register and the competent care court can inspect it.
At the end of the procedure, you will receive a confirmation of registration and the ZVR card.

If you would like to apply for registration in writing:

Download the form for private individuals online and print it out.
Fill out the form, add the supplementary form if required, and send the application documents to the Central Pension Registry.
After you submit the application for registration in writing, the Central Precautionary Registry will send you a data control sheet and an invoice. From the data control sheet, you can see the data to be entered and still make any necessary corrections.
Once you have paid the applicable fee, your health care proxy or care directive will be registered in the Central Care Register and the competent care court will be able to inspect it.
At the end of the procedure, you will receive a confirmation of registration and the ZVR card.

Note: If you have the health care proxy or care directive notarized or certified by a notary public, he or she can arrange for it to be registered in the Central Register of Health Care Providers on your behalf. The same applies if you set up the health care proxy or care directive with the assistance of a lawyer, care association or care authority that is registered with the Central Care Register as a so-called institutional user.
